This is a preview page for the Oblate Day 2026 registration form. The real form will appear on an event page once the details here are approved. Please note a few parts of the form that are ‘responsive’ and will change based on what the user puts into the form:

• The option to choose a double or single room will only appear if the user selects “Stay at Benet House”

• The option to include a roomate’s name will only appear if the users selects “Stay at Benet House” and “Shared Room”

• There is no prompt telling Zoom/Virtual attendees to email Sr. Stefanie for the Zoom link. Sr. Stefanie will automatically receive an email every time someone signs up as a Zoom attendee and needs the link.

• The mailing instructions for checks will only appear if the user selects “Mail a Check” as their payment option.
